PRIMARY ACCOUNTABILITY: Performs eligibility assessments, breastfeeding promotion and nutritional education
PRIMARY DUTIES AND RESPONSIBILITIES
1) Performs eligibility assessment for Women, Infant and Children (WIC) Program
Obtain information on income, family status, and other factors to determine client eligibility for WIC services.
Interview clients and clients’ caretaker to obtain medical and health history;
Determine nutritional risk factors for eligibility for each client using State WIC assessment questionnaire
Identify high-risk participants by assessing client and refer to nutritionist for additional Counsel client on medical and nutritional findings to participant and explain the importance of health care and proper nutrition;
Encourage client participation in education provided by WIC;
Provide basic nutrition education;
Perform complete reassessment of clients’ needs and eligibility every 12 months as required by State.

Promote and support breastfeeding
Complete of annual Civil Rights training
Complete Quarterly time studies as required
Participate in Monthly in service training and annual training as required
Prescribe to client appropriate food packages with adjustments as needed based on nutritional status of client;
Notify of changing status (eligible, ineligible, certified, re-certified, terminated, suspended) in a timely manner;
Inform WIC applicant of WIC client eligibility criteria;
Provide WIC checks and instructions on the use of the checks
Ensure separation of duties
Offer opportunity to vote
2) Establishes flow sheet care plan for certification period
Schedule, and implement secondary nutritional education appointments and other follow-up appointments;
Schedule Registered Dietitian (RD) appointments for high risk clients
Document any; reschedule appointments, make pre-appointment calls
Ensures second contact requirements are planned based on assessment
Schedule follow-up appointments for Peer counselor
Document client progress notes
