Join our dedicated case management team! This is a fun job providing support for clients living in their own homes. The Client Services Coordinator completes the initial intake process and provides ongoing support and follow up for our clients. Establishes and maintains an effective liaison between the client, family, caregivers and case management team.
Provides nursing assessment visits as well as treatments following physicians orders under our Home Health License.
Reviews daily care notes to assure appropriate care is being provided and plan of care is being followed. Provides support, instruction and supervision of care staff. Acts as an advocate for our clients and provides additional resources as needed.
Monitors overall conditions of homes to maintain a safe environment for clients and caregivers.
Participates in case review meetings and offers suggestions as appropriate. Ensures all necessary documentation is up to date and reflects current care needs of our clients.
Represents the company in a positive, professional manner.
Along with case management team, ensures compliance with health department regulations, laws and company policies.
Requires current Registered Nurse license in good standing. Prefer experience working with Seniors
